Office of the Registrar
Verification requests, including transcript requests, must be completed and signed by the student
and submitted to the Office of Student Affairs. Forms can be faxed, mailed, or hand delivered.
Submission of incomplete forms may delay processing. The forms must be physically signed.
* Please note, name change requests must be accompanied by official documentation
of the name change, i.e. copy marriage license.
